Here’s how to complete CR35 – ABC Data Collection Form<\/font>:<\/p>\n\n\n\n

Step 1. Next, click CLIENTS (PWS)<\/strong>, You will see a list of PWS that you are supporting. Choose the PWS you supporting during the medical appointment by clicking their name.<\/p>\n<\/div>

Step 2. Click on Care Management <\/strong>from the top bar. Then click Client Forms <\/strong>from the left sidebar as shown Finally, click the button in the top right corner called +Add Client Form<\/strong><\/p>\n<\/div>

Step 3. After clicking +Add Form<\/strong>, you will see the popup box shown. Ensure that the correct Service* <\/strong>is selected (usually Group Living). Under Service Form*<\/strong> select CR35 – ABC Data Collection Form <\/strong>and click Start Filling<\/strong>. A new window will open and load the form to fill out<\/p>\n<\/div>

\u2022Once the form has loaded, fill in all required fields marked with an asterisk *<\/p>\n\n\n\n

\u2022The forms will often have more info or context for each field shown in blue text<\/p>\n\n\n\n
