We have arranged several hub-clinics, to meet the interest we received. These clinics are open to the people we support and employees. Please pre-book an appointment to help us limit the number of people waiting, and to take appropriate precautions against COVID. WDGPH will be told the number of bookings and bring only the required number of doses.  They will also bring COVID-19 vaccine if requested.  Please contact me directly via email if anyone requires a first or 2^nd dose of a COVID-19 vaccine. 

We’ve confirmed the following dates and locations with Public Health:

• Monday, November 15^th from 10am-2pm
  Holody Centre for Recreation (8 Royal Rd., Guelph)

• Monday, November 22^nd from 10am-12pm
  Community Living Centre – Fergus (280 St Patrick St W, Fergus)

• Monday, November 22^nd from 2:30-3:30pm
  Community Living Centre – Erin (10 Thompson Crescent, Erin)

• Monday, November 29^th from 11-1pm
  Mount Forest Administration Building (135 Fergus St. S, Mount Forest)

As a reminder, if the person you are supporting requires their SDM to complete the consent form, please be sure to have that completed, and on-hand for their appointment time.
